PDF Word Doc ‘Credit Card Slips’

I ran out of credit card sales slips. Is there a PDF or Word document I could use in the interim?

My order for credit card sales tickets for use in my manual imprinter should come in under a week. Until then, I'm looking for a professional looking form I can print out to record the necessary credit card information and get the card holder's signature to have on file. I guess I could just design one in Word, but I'd prefer something generic and pre made so it looks official! Any suggestions on where I could find something like this? (My google search for 'credit card sales slip transaction ticket' didn't yield anything good.)
